fetch_paper, with repeat suppression’s override exposed.

Before any network, the session’s crate::repeat::RepeatIndex is asked whether this ref was already answered: a terminal or needs_config answer within the replay window returns FetchError::Replayed carrying that answer’s code; a retry_after answer less than the minimum gap ago returns one carrying RATE_LIMITED and the seconds left. opts.force asks anyway, and appends a repeat_forced row saying so.
